Teeth whiteners are oxidizing chemical agents formulated to lighten the color of tooth structure. At their core, they rely on peroxide compounds—either hydrogen peroxide or carbamide peroxide—that break down the chromophoric (color-bearing) molecules embedded in the tooth. This process is not cosmetic surface cleaning. It is a chemical reaction that penetrates tooth structure itself.
To understand why this matters, you need to know what you're actually trying to change. A tooth's color is determined by the dentin beneath the enamel (which is naturally yellow and becomes more yellow with age) and by stains trapped inside the enamel or on its surface. These stains fall into two categories: extrinsic and intrinsic. Extrinsic stains sit on the enamel surface—coffee, tea, wine, tobacco, and certain foods deposit pigment that builds up over years. Intrinsic stains are embedded inside the enamel itself, caused by certain medications (tetracycline), excessive fluoride during tooth development, trauma, or simply the natural darkening of dentin as teeth age. A teeth whitener's effectiveness depends entirely on which type of stain you're dealing with.
Extrinsic stains are the easiest to lighten. Because they sit on the surface, peroxide molecules reach them quickly, and the oxidation process is straightforward and relatively fast. A patient who drinks coffee daily and has built up surface staining will often see dramatic results in days.
