Chapter 1: Understanding Surgical Gowns
A surgical gown is not a garment in the traditional sense. It is an engineered barrier device—a precisely constructed assembly of materials designed to prevent fluids, blood, and pathogenic microorganisms from passing in either direction between the surgical team and the patient. Understanding this distinction is the foundation for every subsequent decision about selecting, using, and sourcing surgical gowns.
In the operating room, the surgical gown serves as the primary physical barrier between a sterile field and contamination sources. The surgeon and surgical team, having scrubbed and donned sterile gloves and a surgical gown, are considered "sterile" for the duration of the procedure. The gown's job is to maintain that sterile status by preventing contamination from non-sterile sources—typically the healthcare worker's skin and underlying clothing—from reaching the patient, and simultaneously preventing blood and bodily fluids from the patient from reaching the healthcare worker's skin.
This dual-direction barrier requirement is crucial. A gown that blocks fluid from reaching the worker but allows microorganisms to pass through the dry fabric in the opposite direction is failing its primary function. Conversely, a gown that stops bacterial penetration but allows large volumes of blood to strike through and wet the underlying clothing has also failed—because saturation creates a pathway for fluid-borne pathogens to migrate. Neither direction can be neglected.
