Chapter 1: Understanding Respiratory Care

Respiratory care is the application of specific therapies and devices to support or replace a patient's natural breathing function when it is impaired by disease, trauma, or surgery. It is a field of medicine, but in this handbook it is also a field of engineering, logistics, and commerce. You might be a respiratory therapist deciding on a ventilator mode for a patient with ARDS, a home-care company manager evaluating a new fleet of oxygen concentrators, or a biomedical technician troubleshooting a high-flow nasal cannula system. In every case, your work revolves around a defined set of physical principles and clinical goals. The moment you understand that, the equipment stops being a box with a screen and becomes a tool for managing a patient's gas exchange, work of breathing, and lung mechanics.

At the core of all respiratory care is the need to deliver an appropriate concentration of oxygen to the blood and remove carbon dioxide from it. This exchange happens at the alveolar-capillary membrane in the lungs. While a clinician may talk about "oxygen saturation" or "partial pressure of carbon dioxide," the equipment you use is simply a mean to an end: to maintain that exchange within a physiologically safe range. For a patient with COPD, their airways are narrowed, making it harder to move air. For a patient with pneumonia, the alveolar space is filled with fluid, limiting the surface area for gas exchange. For a patient under anesthesia, their respiratory drive is suppressed.
