Drug testing is not a single procedure. It is a family of chemical and biological methods used to detect the presence of drugs or their metabolites in human samples—typically urine, blood, saliva, or hair. Whether you work in occupational health, run a laboratory, enforce legal compliance, or manage a workplace safety program, you will encounter different test types used in different contexts, often producing results that seem contradictory or unclear. Understanding what a drug test actually measures, how it detects what it's designed to detect, and why it sometimes produces false results is foundational to using tests correctly and interpreting them with confidence.
What Drug Tests Measure
A drug test measures the presence of a specific substance or its chemical breakdown products—called metabolites—in a biological sample. When someone uses a drug, their body absorbs it, processes it through metabolism, and excretes most of it through urine, sweat, saliva, or other routes. A drug test doesn't measure impairment or current intoxication in most cases; it measures exposure. A person who used marijuana three weeks ago and is now completely sober will still test positive on a standard urine test because THC metabolites remain detectable long after the drug's psychoactive effects have worn off.
This distinction matters enormously. A positive result on a urine test means "this person has a detectable level of a drug or its metabolite in their urine at the time of collection." It does not mean "this person is currently high" or "this
