Chapter 1: Understanding Dental Cement
Dental cement is the unsung workhorse of modern restorative dentistry. It is the thin layer of material that creates a permanent or temporary bond between a prepared tooth and a restoration or appliance. While it is often invisible in the final result, its failure is rarely invisible, it is the primary cause of debonded crowns, painful post-operative sensitivity, and recurrent caries. This chapter is about defining what this material actually is, how its fundamental properties determine its clinical behavior, and why a deeper understanding of it is a competitive advantage for any practice or lab.
The Material Reality: More Than Just Glue
To understand dental cement, it is helpful to first understand what it is not. It is not an ordinary adhesive like cyanoacrylate or epoxy, which are polymers invented for industrial bonding. Dental cement is a specialized group of materials that must meet a demanding, and often contradictory, set of clinical requirements. It must be biocompatible with living pulp tissue, but strong enough to hold a ceramic crown under 500 pounds per square inch of chewing force. It must flow into a thin film to allow the crown to seat properly, but have enough viscosity to prevent it from washing out from under the restoration. It must bond to a variety of surfaces, including enamel, dentin, precious and non-precious metals, porcelain, and composite resin, none of which share the same chemical properties.
Consider the clinical scenario of cementing a full-coverage gold crown on a
