Chapter 1: What Does a Physical Therapist Actually Do?

You already know the elevator pitch: physical therapists are healthcare professionals who help people recover from injuries and move better. That’s true, but it’s about as useful as saying a chef “makes food.” So let’s go deeper. What does a PT actually do from the moment they clock in to the moment they leave? What fills the eight to ten hours between?

The short version: a PT is part clinician, part teacher, part documentarian, and part billing clerk. They evaluate, diagnose, and treat problems with movement β€” but the treatment session is only half the job. The other half is charting what you did, justifying it to an insurance company, and planning what you’ll do next.

The Core of the Job: Evaluation and Treatment

Every patient relationship starts with an evaluation. This is a 45- to 60-minute deep dive into why a person is in pain or can’t move the way they need to. You’ll ask questions β€” a lot of them. When did the pain start? What makes it worse? What makes it better? What do you want to get back to doing? Then you’ll move them. You’ll watch how they bend, squat, reach, and walk. You’ll test their range of motion with a goniometer β€” a fancy protractor for measuring angles at joints. You’ll test strength with manual resistance β€” pushing against their limbs to gauge power. You’ll feel for tender spots with your hands, a technique called palpation.
